Qualifications

• Bachelor’s degree in social work, psychology, nursing, or human services field.
• Two years of higher education with two years of experience in psychiatric, substance use treatment or developmental disabilities.
• Four years of equivalent experience with consumers and their families in psychiatric, substance use treatment or developmental disabilities services.
• High school diploma or equivalent and applicable training and testing as required by the Department of Mental Health. Lived experience with behavioral health and/or co-occurring treatment and justice involvement.
• A mental health professional, OR

Responsibilities

• Identify strengths and goals of person(s) served.
• Assist person(s) served in making independent choices and being proactive in treatment.
• Assist person(s) served in overcoming barriers to Treatment Court completion and community reintegration.
• Ensure access to behavioral health services including therapy, medication management, and related healthcare.
• Link person(s) served with community resources to obtain housing, insurance, and gainful employment.
• Provide education about managing symptoms of substance use disorder and/or mental illness.
• Help person(s) served develop interpersonal and independent living skills.
• Provide transportation for essential treatment interventions.
• Attend and participate in treatment team meetings.
• Develop and implement outreach strategies and referral pathways with project staff.
• Gather relevant data and generate reports as needed.
• Communicate information effectively in oral and written form.
• Complete required documentation in a timely manner.
• Adhere to evidence-based practice models and agency policies.

Education

Bachelor’s degree in social work, psychology, nursing, or human services field
