Formación

• High School diploma or GED required or equivalent.
• Experience in mental health or social services preferred.
• Strong case management and communication skills.

Responsabilidades

• Complete client assessments and develop care plans with goals and objectives.
• Provide weekly case management and skills training to clients.
• Deliver recovery coaching in home or community settings.
• Coordinate with internal and external treatment teams.
