Job summary

Root Center for Advanced Recovery in Hartford, CT is seeking a Recovery Coach to support clients on their recovery journeys, serving as a guide and role model.

You will maintain caseloads, help clients build life skills, coordinate transportation to services, and foster community connections to promote independence.

Qualifications

• High school diploma or GED and at least 1 year experience in medical or mental health or related area.
• CCAR Certification or Advocacy Unlimited Certification required.
• Experiential knowledge of Medication Assisted Treatment (MAT).

Position Summary

Under the supervision of the Clinical Supervisor, the Recovery Coach is responsible for providing recovery services to clients based on the consumers needs and preferences. The role is to serve as "recovery guide" and role model for persons served. When seeking to re-engage patients who meet program criteria, the worker will serve as a linkage with the counselor and as a support for the patient completing community referrals. Independence in the community is the ultimate goal of the program.

Hourly Rate Range: $20.00 - $25.00

Essential Functions

Maintains a caseload of Intensive Case Management clients and is responsible for assisting them in various recovery life skills consistent with their needs and preferences:

Examples of skill areas include, but are not limited to:

• Guidance as to personal care & independent living skills
• Personal health & safety awareness
• Money management
• Transportation
• Interpersonal communications

The Recovery Coach will assist the client in successful completion of their recovery plan by:

• Monitoring client’s progress in completing recovery goals.
• Coordinating transportation for patients to necessary community services.
• Attending community meetings to market the program and to develop community resources.
• Advocating to ensure patients have access to and receive services from community-based programs.

Minimum Qualification Requirements

High school diploma or GED and at least (1) year experience in medical or mental health or a related area required. CCAR (Connecticut Community for Addiction Recovery) Certification or Advocacy Unlimited Certification required . Experiential or actual knowledge of Medication Assisted Treatment (MAT).
