Job summary

Valley Hope of Atchison is seeking a Case Manager to join our residential treatment team in Atchison, KS. You will plan continuing care, facilitate recovery sessions, and coordinate housing, education, and medical resources for patients and families.

You will educate patients and families about substance use disorders and prevention, collaborate with external partners, and maintain accurate documentation and discharge materials as part of our commitment to high-quality care.

Qualifications

• One or more years of experience in a similar position and/or healthcare environment.
• CPR certification is required.

Responsibilities

• Plan and assist with continuing care for assigned patients.
• Facilitate patient rehabilitation group sessions and educational workshops on recovery topics.
• Identify recovery resources for housing, education, vocational development, mental health, community support groups and medical care; provide assistance accessing those resources.
• Provide education regarding substance use disorders, mental illness and prevention to patients, family members, and significant others.
• Work with external parties including family members, agencies, managed care organizations, and insurers.
• Record treatment activities and patient observations in records; complete paperwork accurately and timely.
• Complete clinical documentation including discharge materials.
• Communicate with supervisor and clinical staff about patients’ treatment and status.
• Maintain knowledge of counseling procedures and substance abuse treatment per policy and guidelines.
