Valley Hope of Atchison invites you to join a dedicated team as a Case Manager in a residential treatment setting just northwest of Kansas City. You will work with patients to plan and coordinate continuing care, education, and support resources to foster lifelong recovery.

The role requires prior healthcare experience and CPR certification, with a preferred bachelor’s or graduate degree in behavioral sciences, education, or social work, and CM certification in the state of employment.

Qualifications

• One year of experience in a healthcare environment.
• CPR certification.

Responsibilities

• Plans and assists with the continuing care for assigned patients.
• Facilitates rehab group sessions and educational workshops on recovery topics.
• Identifies needed recovery resources and helps patients access housing, education, vocational development, mental health, and medical care.
• Provides education about substance use disorders, mental illness, and prevention to patients and families.
• Maintains accurate treatment records and completes discharge materials.
