Job summary

Valley Hope of Atchison is seeking a Case Manager to join our dedicated team at our residential treatment facility in Atchison, KS. You will work under the Program Director and coordinate continuing care, recovery resources, and family education while ensuring compliant documentation and patient-centered care.

Ideal candidates have 1+ year healthcare experience, CPR certification, and preferably a degree in behavioral sciences or social work; state CM certification is a plus.

Qualifications

• One plus years of experience in a similar position and/or healthcare environment.
• CPR certification is required.

Responsibilities

• Plan and assist with continuing care for assigned patients.
• Facilitate patient rehabilitation group sessions and educational workshops on recovery topics.
• Identify recovery resources for housing, education, vocational development, mental health, community support groups and medical care; provide assistance to patients in accessing those resources.
• Provide education regarding substance use disorders, mental illness and prevention to patients, family members, and significant others.
• Work with external parties including family members, agencies, managed care organizations, and insurance companies.
• Record treatment activities and patient observations in patient records accurately and timely; complete paperwork requirements as needed.
• Complete all clinical documentation including discharge materials in an accurate and timely manner.
• Communicate with onsite supervisor about daily job duties and supervisory direction provided.

EDUCATION & EXPERIENCE:

Required:

• One (1) plus years of experience in a similar position and/or healthcare environment.
• Cardiopulmonary Resuscitation (CPR) certification.

Preferred:

• Bachelor’s or graduate degree in behavioral sciences, education, or social work.
• One (1) plus years of work experience in substance abuse counseling industry.
• Certified as Case Manager in the state of employment.

JOB SUMMARY:

• Works under the supervision and direction of the Program Director.
• Plans and assists with the implementation of continuing care for assigned patients.

• Works with all relevant and interested external parties including family members, agencies, managed care organizations, and insurance companies.
• Records treatment activities and patient observations in patient records accurately and timely; completes paperwork requirements as needed in an accurate and timely manner.
• Completes all clinical documentation including discharge materials in an accurate and timely manner.
• Communicates with onsite supervisor about daily job duties and supervisory direction provided.
• Communicates with clinical supervisors, clinical staff, and medical staff regarding patients’ treatment and status.
• Logs all supervision as per state requirements.
• Maintains and demonstrates a current working knowledge of applicable counseling procedures and substance abuse treatment in accordance with company policy and State and Federal guidelines.
• Adheres to Valley Hope’s clinical philosophies, policies, and procedures as well as any other regulating bodies to ensure quality patient care.
• Participates in all assigned staff meetings, staff development, and training as required.
• Interacts and communicates with others (patients, co-workers, management, clients, and vendors) in a professional and tactful manner including treating them with respect and consideration regardless of their status or position.

WORK ENVIRONMENT:

• Residential treatment facility setting; requires walking throughout the facility.
