Qualifications

Responsibilities

• Meet with caseload daily to assess needs and progress.
• Conduct biopsychosocial and Columbia assessments within 48 hours of admission.
• Prepare treatment and discharge plans in collaboration with clients and supports.
• Facilitate groups as assigned.
• Maintain documentation and notes per policy.
• Coordinate with referral sources and community resources.
