Job summary

Gosnold in Massachusetts is seeking a BH-JI/CSP Case Manager to support patients with mental health and addiction recovery services. The role includes caseload management (17–20 patients), coordinating with community resources, and documenting care plans in a CRM system.

Salary ranges from $22.00 to $23.00 hourly. Requires a Bachelor’s degree in counseling, psychology, social work, nursing, or related field and two years’ experience in mental health/addiction services, plus a valid MA driver’s

Qualifications

Bachelor’s degree in counseling, psychology, social work, nursing, or related discipline required.
Two years of professional or lived experience in mental health and addiction.
Strong interpersonal and organizational skills; comfortable with the justice system engagement.
Valid, clear MA driver’s license.

Responsibilities

Conduct patient interviews and basic assessments and engage patients into treatment.
Develop and manage case management service plans.
Coordinate with community resources and houses of correction as needed.
Maintain patient records per Gosnold standards.
Collaborate with a multidisciplinary team and participate in supervision.
Transport patients to resources when required and coordinate transportation resources.
