Job summary

Merakey is seeking a Drug & Alcohol Case Manager in Philadelphia to provide case management, service planning, advocacy, and resource coordination for up to 30 clients in a recovery-focused program.

The role involves coordinating care, ensuring compliance with regulations, and maintaining detailed client records. On-call duties may be required, with work hours Monday through Friday from 7:00am to 3:30pm or 7:30am to 4:00pm.

Qualifications

• High school plus 12 credits of higher education coursework in psychology, addictions studies, social work or related field with specialty or concentration in counseling.
• One to three years case management experience in behavioral health.
• Must be able to work Monday-Friday 7:00am -3:30pm or 7:30am - 4:00pm.
• Valid Driver's License.

Responsibilities

• Serve as primary case manager for assigned clients and develop person-centered service plans.
• Coordinate care and monitor client progress, connecting clients to recovery, medical, housing, and employment resources.
• Maintain client records, treatment plans, progress notes, and required documentation.
• Participate in supervision, team meetings, and quality improvement activities.
