Job summary

WestCare Tennessee is seeking a dedicated Case Manager to join the Recovery team. You will work with Treatment Counselors, Supportive Employment Coaches, and Peer Specialists to serve individuals and coordinate referrals that align with each client’s ISP and treatment plan.

The role requires a four-year degree related to counseling or social services and involves documenting services, conducting follow-ups, and contributing to program outcomes in a dynamic, community-focused environment.

Qualifications

• Bachelor's degree or higher in Addiction Counseling, Psychology, Social Work, or related field.
• 2–4 years of related project field experience may substitute for the degree.
• Strong case management and documentation skills.

Responsibilities

• Collaborate with treatment and support staff to coordinate services for clients.
• Deliver evidence-based curriculum to individuals in a group setting.
• Educate and refer clients to community resources for housing, food, employment, transportation, etc.
• Assist clients in building or restoring support systems in the community.
• Advocate to ensure access to needed services for clients.
