Bay Cove Human Services is seeking a Case Manager for Diversionary Services at Andrew House in Boston. The role involves working with a multidisciplinary team to provide counseling and community referrals to individuals with substance use and co-occurring disorders.

The position is non-exempt with a Friday and Saturday 7am-3pm schedule, focusing on intake, assessment, and care coordination, with an emphasis on individualized recovery plans and crisis support.

Qualifications

• Bachelor's degree in a social work or related field or equivalent experience.
• High School diploma or equivalent is required.
• Minimum of 1 year experience in Behavioral Health or Human Services.
• Working knowledge of Windows, Word, and Google Email/Apps required.

Responsibilities

• Provide support, counseling, and community referrals to individuals with substance use and co-occurring disorders.
• Engage patients in information gathering to inform psychosocial assessment and treatment goals.
• Coordinate services with external treatment providers and assist patients to coordinate appointments.
• Develop aftercare plans and coordinate transitions to aftercare services.
• Document all services in the Electronic Health Record and communicate changes to the team.
