Job summary

Ivory Plains Recovery Center is seeking a dedicated Case Manager to provide case management across units, coordinate care from admission through discharge, and support individualized treatment plans at our Adair, Iowa facility.

You will work with the treatment team to ensure clients receive coordinated services and transition plans, maintaining ongoing communication with referral sources and providers. Onsite role with competitive compensation and benefits.

Qualifications

• One or more years of related behavioral health case management experience.
• Experience with discharge planning and service coordination.
• Experience with coordinating across multiple providers and teams.
• Experience with documentation and client file maintenance.

Responsibilities

• Develops and implements activities within scope of practice aligned to ISP.
• Collaborates with client and family to implement an effective service plan and explain options.
• Maintains comprehensive clinical records including assessments and progress notes.
• Works with the clinical team to coordinate care with providers and community resources.
• Ensures discharge planning begins at admission and collects collateral input.
• Coordinates referrals and ensures services on the service plan are evaluated monthly.
• Manages screening/assessment for financial entitlements (AHCCCS, SSI/SSD) for caseload.
• Develops and implements transition, discharge, and aftercare plans prior to discontinuation of services.
• Maintains contact with referral sources and treatment providers within designated timeframes.
• Employs crisis intervention techniques as needed.
• Communicates client information to the treatment team and participates in team discussions.
