Job summary

Pinnacle Treatment Centers, Inc. seeks a dedicated Case Manager to provide and coordinate case management and care for residents in treatment.

You will partner with the Clinical Director and Executive Director to maintain a therapeutic environment and ensure resources are used effectively. Responsibilities include coordinating assessments, treatment planning, crisis intervention, discharge planning, and linkage to community services.

Qualifications

• Bachelor’s degree in the Human Service field from an accredited college or university.
• Valid/Current Driver's License.
• Ability to travel up to 15% may be required.

Responsibilities

• Providing case management and care coordination services for patients enrolled in treatment.
• Coordinating assessments, treatment planning, and crisis interventions as needed.
• Facilitate linkages to community resources and assist with service access.
• Coordinate discharge activities and follow-up to ensure continuity of care.
