Job summary

Behavioral Health Services is seeking an experienced Case Manager for the South Bay Family Recovery Center in Gardena, CA. This part-time, day-shift role focuses on case management, recovery support, and care coordination for clients in assigned programs.

You will conduct screenings, develop care plans with clients, and help with discharge planning, housing navigation, and employment support, while maintaining compliance and thorough documentation.

Qualifications

• Bachelor's degree in related field (preferred); AA with related experience acceptable.
• California driver's license and liability insurance required.
• Experience in substance use treatment and case management preferred.

Responsibilities

• Conduct substance use screening, care planning, and treatment activities.
• Develop individualized care plans with clients and treatment team.
• Provide relapse prevention, discharge planning, and recovery support.
• Offer case management including benefits, housing navigation, and employment support.
• Coordinate with medical providers and social services.
• Maintain accurate treatment records and progress notes.
