Job summary

Iris House: A Center for Women Living with HIV Inc. seeks a Case Manager (CASAC/CASAC-T) to provide recovery-oriented case management, assessments, and crisis intervention in a supportive housing setting.

The role includes weekly workshops, collaboration with healthcare and social services, and a focus on trauma-informed, harm-reduction approaches to promote housing stability and wellness. Bilingual English/Spanish preferred.

Qualifications

• Bachelor’s degree in Human Services, Social Work, Psychology, or a related field preferred.
• Current CASAC or CASAC-T credential required.
• Candidates actively completing the requirements for CASAC certification will be considered, subject to verification of their progress and eligibility.
• Two or more years of case management experience preferred.
• Bilingual English/Spanish skills preferred.

Responsibilities

• Maintain a full-time caseload of no more than 20 clients and provide individualized case management.
• Conduct comprehensive intakes and assessments to identify needs across health, HIV care, mental health, and social determinants.
• Facilitate referrals to substance use and mental health treatment services, and coordinate access to care.
