Job summary

Wooded Glen Recovery Center in Henryville, Indiana, is seeking a Case Manager to coordinate care across treatment teams and support discharge and aftercare planning. The role emphasizes collaboration with clients, families, and providers to ensure the individualized service plan is implemented.

As an onsite, non-remote position, the Case Manager will maintain client records, coordinate referrals, and participate in quality care monitoring to promote recovery and continuity of care.

Qualifications

• High School Diploma or equivalent required; Bachelor's degree preferred.
• 1+ year related experience in a behavioral setting preferred.
• Experience with the client population served preferred.
• Experience in case management preferred.

Responsibilities

• Develops and implements activities and programs within scope of practice per ISP.
• Collaborates with client and family to implement an effective service plan, explaining options.
• Maintains the client’s comprehensive clinical record and progress toward recovery.
• Coordinates care with clinical team to ensure ISP is implemented.
• Ensures discharge planning begins at admission with collateral input.
• Coordinates referrals and updates service plan monthly; documents activity.
