Job summary

Wooded Glen Recovery Center located onsite in Kentucky is seeking a Case Manager to coordinate care across units and ensure clients receive services per their individualized treatment plan. You will lead discharge planning and aftercare coordination with providers and families, ensuring timely communication and comprehensive record-keeping.

The role requires a High School Diploma (Bachelor’s in Social Work or related field preferred) and prior behavioral health/case management experience.

Qualifications

• High school diploma or equivalent is required.
• Bachelor’s degree in Social Work, Counseling or related field preferred.
• Experience in behavioral health settings preferred.
• Prior case management experience preferred.

Responsibilities

• Develops and implements activities within scope of practice.
• Implements services per the individualized service plan (ISP).
• Collaborates with client and family to implement effective service plan.
• Maintains comprehensive clinical records and progress notes.
• Coordinates with clinical team to implement ISP across providers.
• Begins discharge planning at admission and obtains collateral input.
• Ensures referrals for identified services are made and coordinated.
• Screens and assesses clients for financial entitlements and AHCCCS applications.
• Develops transition, discharge and aftercare plans prior to discontinuation of services.
• Maintains timely contact with referral sources and providers.
• Uses crisis intervention techniques as needed.
• Communicates client information to the treatment team and participates in goal discussions.
