Wooded Glen Recovery Center in Henryville, Indiana is seeking a Case Manager to coordinate care across providers and support clients per their individualized service plan. This on-site role requires collaboration with the treatment team and family to ensure effective discharge planning.

You will maintain clinical records, complete referrals to AHCCCS/SSI as eligible, and contribute to ongoing quality care, with emphasis on early discharge and aftercare planning.

Qualifications

  • High School Diploma or equivalent is required.
  • Bachelor’s degree in Social Work, Counseling or related human service field preferred.
  • One or more years of related behavioral health experience preferred.
  • Experience with case management is preferred.

Responsibilities

  • Develops and implements activities and/or programs within their scope of practice.
  • Implements services to clients per their individualized service plan (ISP).
  • Coordinates care with client, family and providers to implement the service plan and discuss options.
  • Maintains comprehensive clinical records including assessments, services, coordination of care and discharge planning.
  • Collaborates with the clinical team to ensure ISP services are implemented and coordinated with providers.
  • Ensures discharge planning begins at admission and AHCCCS/SSI/SSD referrals are coordinated.
  • Develops transition, discharge and aftercare plans prior to discontinuation of behavioral health services.
  • Documents client contact with referral sources and other providers within designated timeframes.

Education

About the Job: PURPOSE STATEMENT

The Case Manager supports the department by providing case management to the different units within the facility. As a member of the treatment team, is responsible for coordinating care across multiple providers to ensure clients receive support identified within the individualize treatment plan. Responsible for the overall discharge and aftercare planning needs and placement of the client. Maintains ongoing communication with the treatment team. Maintains routine contact with referral sources and providers while maintaining resource file. Participates in quality care monitoring for the department.

Roles and Responsibilities

ESSENTIAL FUNCTIONS: Develops and implements appropriate activities and/or programs within their scope of practice. Implements services to clients in accordance with their individualized service plan (ISP). Collaborates with the client and client’s family or significant others to implement an effective service plan, explaining the available options to the team, including the advantages and disadvantages of each option. Maintains the client’s comprehensive clinical record, including documentation of activities performed as part of the service delivery process (e.g., assessments, provision of services, coordination of care, discharge planning) and client’s progress toward their recovery. Works collaboratively with the clinical team to engage, educate, communicate, and coordinate care with client, their family, behavioral and general medical health care providers, community resources and others to ensure that all services prescribed in the individualized service plan (ISP) are implemented. Assures that discharge planning begins at admission. Obtains collateral input from providers and supports. Ensures all appropriate referrals for identified services on the service plan are made and coordinated with service providers via contracted network providers or community resources; Ensures all covered services identified on the service plan are evaluated and updated monthly. Coordinates referral or completes the screening and assessment on all clients on caseload for financial entitlements (AHCCCS, SSI/SSD etc.); completes AHCCCS applications on all clients on caseload meeting criteria. Ensures the development and implementation of transition, discharge, and aftercare plans prior to discontinuation of behavioral health services. Ensures all contact with referral source and/or other treatment providers is done within designated time frame and documented accordingly. Employs appropriate crisis intervention techniques as needed. Communicates appropriate client information to the treatment team; participates in team discussion and evaluation of client goals, objectives and motivations.

Education/Experience/Skill Requirements

High School Diploma or equivalent required, Bachelor’s degree in Social Work, Counseling or related human service field preferred. One or more years previous related experience in a behavioral setting preferred. Previous experience with the client population served by the facility preferred. Previous experience in case management preferred.

Licenses/Designations/Certifications

CPR certification and de-escalation training required (training available upon hire and offered by facility). First aid may be required based on state or facility requirements.

Work Location

This position is onsite at the facility and is not a remote position.

Supervisory Requirements

This position is an Individual Contributor.
