Job summary

Avenues Recovery Center Bucks is seeking a compassionate Case Manager to support clients through intake, treatment, and discharge planning. You will advocate for clients, initiate intake-based case management, and monitor insurance coverage to ensure seamless access to services.

You will coordinate housing, financial aid, and legal/vocational supports, and lead discharge planning to secure continuing care and sober housing options post-treatment.

Responsibilities

• Advocate for client, addressing and resolving any external barriers to treatment.
• Initiate case management for each client upon intake, addressing any pressing needs.
• Oversee client accounts, ensuring insurance is active throughout duration of treatment.
• Provide ongoing case management throughout the duration of treatment.
• Arrange housing, financial aid, legal aid, vocational training etc. as necessary for client post-discharge.
• Facilitate comprehensive discharge planning (continuing care, housing, sober support etc.).
