Job summary

Crossroads is seeking a Case Manager to support the Clinical Coordinator in delivering client services within Arizona-licensed substance use treatment programs. You will manage treatment planning, case load, and discharge planning, coordinating care across teams.

The role requires strong documentation, communication, and adherence to regulatory requirements. It includes crisis intervention, staff collaboration, and travel between sites as needed.

Qualifications

• Must meet Arizona Department of Health Services personnel member requirements: at least 21 years old or licensed/certified under A.R.S. Title 32.
• Current CPR/First Aid/Narcan training.
• Current and annual negative TB Test.
• Current and valid Fingerprint Clearance Card.
• Clean Motor Vehicle Record (MVR) check.
• Pass a background check and ADHS Adult Protective Services check.

Responsibilities

• Develops and provides individual treatment plans; monitors, evaluates and documents client progress.
• Maintains a client caseload and completes required documentation in a timely manner.
• Provides individual and group interventions including crisis management.
• Facilitates coordination of care and discharge planning.
• Participates in clinical staff meetings and trainings.
• Assist clients with government assistance applications and referrals to agencies.
