Job summary

Qualifications

• Bachelor’s degree preferred.
• Minimum of 2 years in behavioral healthcare or substance abuse recovery.
• Experience in group facilitation and case management.
• Knowledge of utilizing electronic health record software.
• Knowledge of ASAM criteria.
• Strong written and verbal communication skills.

Responsibilities

• Coordinate care and case management for assigned caseload.
• Communicate with Counseling and UR teams on patient care aspects.
• Maintain patient caseload and document coordination of care weekly.
• Complete aftercare planning, discharge planning, and required contacts per policy timelines.
• Maintain HIPAA and 42 CFR Part 2 compliance in all documentation.
