Job summary

PCC Community Wellness Center seeks a Recovery Social Work Care Manager (CADC-preferred) to coordinate screening and interventions for patients with complex chronic care needs at the ARC Recovery Services Clinic in Chicago. This licensed social worker connects patient needs with PCC ARC and hospital resources to support whole health outcomes.

The role collaborates with a multidisciplinary team, provides behavioral health support, and maintains care coordination across settings.

Qualifications

• Graduate of an accredited school of social work.
• 2–3 years clinical experience preferred.
• 2–3 years EMR experience preferred.
• Proficient in MS Office applications.
• Excellent communication and teamwork with healthcare providers.

Responsibilities

• Coordinate screening and intervention for patients with complex chronic care needs.
• Serve as a connector between patient needs and PCC ARC/Hospital resources.
• Collaborate with interdisciplinary teams to support whole health outcomes.
• Maintain patient registry and ensure follow-up on referrals to ensure continuity of care.
