Job summary

Gaudenzia, Inc. is seeking a Specialty Care Coordinator for Wellness Village on a part-time basis in Philadelphia, PA. The role focuses on intake, care coordination, and transitioning clients through different levels of care within our residential and recovery services.

Responsibilities include coordinating treatment plans, liaising with clinical staff, and ensuring proper documentation and referrals. A minimum Associate Degree or CAC certification is required, with several years of related

Qualifications

• Education and Training: A minimum of an Associate Degree or full certification as an Addictions Counselor
• Experience: Prior work in addictions counseling or client care coordination is preferred
• Knowledge: Understanding of recovery services, HIPAA, and confidentiality requirements
• Ability: Strong communication and ability to work in a team-based setting

Responsibilities

• Conduct intake interviews and orientation to care coordination prior to discharge from residential care
• Liaise with clinical staff for review and verification of services
• Complete Recovery Plan with client and primary counselor
• Identify health care and recovery needs and initiate referrals
• Assist clients in accessing next levels of care and remove barriers
• Develop and maintain relationships with services and programs
• Maintain care coordination schedule and client records
• Ensure timely follow-up and documentation in EHR
