Qualifications

• Or an Associate degree in a social science with lived recovery experience.
• Ability to interpret regulations, policy and procedures and apply them accordingly.
• Ability to work with persons with physical, mental, or emotional disabilities or who are economically disadvantaged or who are involved with the criminal justice system.
• Establish and maintain effective working relationships with staff, outside agencies and the public.
• Travel is required between offices.

Responsibilities

• Advocate for and assist patients with physical and mental health needs.
• Develop partnerships within the community for patient-centered care.
• Coordinate access to addiction treatment and ancillary services (housing, childcare, medical, financial).
• Maintain confidentiality and privacy of patient information.
• Monitor treatment progress and participate in quality improvement activities.
• Communicate with care teams and external agencies to coordinate services.
• Travel between offices as required.
• Attend trainings and stay current with COE standards and best practices.
