Job summary

CLARE|MATRIX seeks a full-time Care Coordinator (Case Manager) in Santa Monica, CA to link participants with health and social services. You will work with a multidisciplinary team to address housing, employment, education, financial, legal, medical, and recovery needs, ensuring access to needed resources and successful reintegration into the community.

You will conduct assessments, coordinate transitions within the SUD system, and document progress and outcomes in the electronic health record.

Qualifications

• Registration or certification with a state approved AOD certifying body.
• One year of experience in social services or related field preferred.
• Strong oral and written communication skills.
• Must have a valid CA driver's license.

Responsibilities

• Registration or Certification with a state approved AOD certifying body. Must be registered or certified upon hire.
• In collaboration with the treatment team, assess participants to address problems in Housing, Employment, Education/Training, Financial, Legal, Sobriety/Social, SUD Treatment, Family/Social, Medical/Psychiatric.
• Conduct in-home, in-field and on-site assessments and care coordination/linkages.
• Assist the primary counselor with discharge-planning efforts, including housing, employment, medical/psychiatric, and aftercare planning.
• Provide participants with referrals and linkages for resources and services identified in the Treatment Plan/Problem List.
• Coordinate care with community agencies by assisting with eligibility, documentation, and applications.
• Follow up to ensure appointments and linkages are completed and progress is tracked.
• Communicate with participants, staff, and providers via telephone, email, and progress notes; provide weekly updates.
