Job summary

Central City Health in Detroit (MI) is hiring a Peer Support Specialist to work in our integrated care team. You will help clients navigate primary care, behavioral health, housing services, and community resources, while modeling recovery and supporting daily living skills.

This is a full-time, in-person position offering benefits and opportunities to advance, with a requirement for Michigan Peer Support Specialist Certification and a valid driver’s license.

Qualifications

• High School Diploma or GED required.
• Peer Support Specialist Certification required.
• Michigan Chauffeur’s Driver’s License required; license can be acquired within 30 days if not held.
• BLS or CPR certification preferred.
• Experience with mental health patients and cooperative team work preferred.

Responsibilities

• Ensure actions and communications reflect the organization professionally.
• Conduct outreach to individuals needing a medical home or with chronic disease.
• Help patients access primary care and community services; coordinate with treatment team.
• Determine eligibility and enroll uninsured or underinsured patients; assist with Care Link enrollment.
• Provide recovery education across stages of recovery.
• Assist patients in creating wellness plans and set goals with supports.
• Link individuals to resources and encourage community involvement.
• Teach problem-solving techniques and coping skills.
• Model recovery and support patient empowerment; promote health literacy.
