Job summary

Lincolnshire County Council is seeking a Community Care Officer to join the Hospital Social Care Team. The role focuses on assessing care needs, coordinating personalised packages, and supporting people returning to independent living after hospital stays.

The position is based at Lincoln County Hospital with travel to NLAG hospitals as needed. It operates on a 30-hour weekly schedule with hybrid working (1 day remote).

Qualifications

  • The role requires a good standard of general education, including English and Maths, with a minimum of four GCSEs at Grade C or above or equivalent.

Job description

Are you looking for a role where no two days are the same?

Our Hospital Social Care Team at Lincoln County Hospital is looking for a Community Care Officer to join us.

You will be part of a busy, supportive multidisciplinary team, working alongside social workers, occupational therapists, nurses, therapists and health colleagues to help people leave hospital safely and return to living as independently as possible.

Hospital-based, but not limited to one hospital.

Although you will be based at Lincoln County Hospital, our team also supports hospital discharges from Northern Lincolnshire and Goole NHS Foundation Trust (NLAG) hospitals, including Grimsby Hospital, Scunthorpe General Hospital, Goole and District Hospital and Diana, Princess of Wales Hospital.

This means that travel to other hospital sites may be required as part of the role.

You will have the opportunity to work across health and social care, building strong professional relationships and helping people move from hospital back into their communities.

When will you work?

This is a 30-hour permanent position.

Our usual working hours are:

Monday to Thursday: 8.45am to 5.15pm

Friday: 8.45am to 4.45pm

The role requires a good standard of general education, including English and Maths, with a minimum of four GCSEs at Grade C or above or equivalent.
