Derive Care Limited provides high-quality, person-centred domiciliary care to support individuals to remain safe, independent and comfortable in their own homes. We are seeking caring, reliable Domiciliary Care Assistants to join our team across Monmouth and Newport, with travel between clients' homes.

A full UK driving licence and access to a reliable vehicle are mandatory. This role offers flexible hours (full-time 35-45 hrs/wk or part-time 15-20 hrs/wk) and comprehensive induction plus

Responsibilities

• Provide person-centred care in accordance with individual care plans.
• Support clients with personal care, dressing, bathing and continence care.
• Assist with mobility, transfers and other daily living activities.
• Support with meals, hydration and medication where trained and authorised.
• Provide companionship and encourage independence and choice.
• Assist with appropriate household tasks related to the client's wellbeing.
• Support clients with appointments, shopping and community activities where required.
• Monitor and report changes in clients' wellbeing or any safeguarding concerns.
• Maintain accurate and confidential care records.
• Follow safeguarding, health and safety, infection control and company procedures.
• Work professionally with clients, families, colleagues and other professionals.
• Full UK driving licence – mandatory.
• Access to a vehicle – mandatory.

• Ability to work independently and as part of a team.

Domiciliary Care Assistant/ Healthcare Assistant

Locations: Monmouth and Cardiff, Wales

Job Type: Full-time / Part-time, Permanent

Reports to: Care Coordinator / Registered Manager

About the Role

Derive Care Limited provides high-quality, person-centred domiciliary care to support individuals to remain safe, independent and comfortable in their own homes.

We are looking for caring, reliable and motivated Domiciliary Care Assistants to join our team across Monmouth and Newport.

This is a community-based role involving travel between clients' homes. A full UK driving licence and access to a reliable vehicle are mandatory.

Part time - 15-20 hrs a week

Full time - 35-45 hrs a week

Key Responsibilities

• Provide person-centred care in accordance with individual care plans.

You will work in clients' homes across Monmouth and/or Newport travelling between visits as required.
