Qualifications

  • Bachelor's degree in Social Work, Public Health, Psychology, Criminal Justice, Human Services, or a related field required; Master's degree preferred.
  • Minimum of three (3) years of experience managing social service, public health, behavioral health, or re-entry programs.
  • Minimum of two (2) years of supervisory or team leadership experience.
  • Experience managing grant-funded programs and ensuring compliance with contractual and regulatory requirements.
  • Knowledge of justice-involved populations, re-entry services, trauma-informed care, housing resources, behavioral health, and community-based services.
  • Experience developing partnerships with community organizations, government agencies, and justice system stakeholders.
  • Strong organizational, project management, and problem-solving skills with the ability to manage multiple priorities.
  • Excellent written, verbal, and interpersonal communication skills.
  • Demonstrated ability to analyze data, prepare reports, and use performance metrics to improve program outcomes.
  • Attention to detail with the ability to ensure accuracy, maintain compliance, and proactively identify and resolve issues.
  • Highly proactive and self-motivated with the ability to anticipate needs and drive projects with minimal supervision.
  • Proficiency in Microsoft Office Suite and EHR systems, preferably eClinicalWorks (eCW).

Responsibilities

  • Oversee the daily operations of assigned Re-Entry Services programs, ensuring high-quality, trauma-informed service delivery.
  • Supervise, coach, and evaluate program staff, promoting accountability and professional development.
  • Monitor program performance, deliverables, and outcomes to ensure compliance with grant, contract, and organizational requirements.
  • Conduct regular chart reviews, documentation audits, and quality assurance activities to ensure compliance with program standards and funding requirements.
  • Monitor program budgets and expenditures, ensuring responsible use of grant funds and adherence to funding guidelines.
  • Prepare and submit timely program reports, performance data, and other required documentation for leadership and funders.
  • Develop and maintain strong partnerships with justice system agencies, community-based organizations, healthcare providers, and other stakeholders to enhance service coordination and referrals.
  • Coordinate outreach, engagement, and community education activities to increase awareness of re-entry services and improve access to care.
  • Collaborate with internal departments to ensure integrated, patient-centered services.
  • Identify operational challenges and implement process improvements to enhance program efficiency and client outcomes.
  • Support recruitment, onboarding, training, and retention of program staff.
  • Participate in leadership meetings, community collaboratives, trainings, and other activities representing St. John’s Community Health.
  • Perform other duties as assigned by the Director of Re-Entry Services.
  • Develop and implement new grant-funded programs and contracts with planning, staffing, budgeting, policy development, and monitoring.
