Job summary

UMMA Health is seeking a Program Manager for the QIF-TJI initiative in Huntington Park, CA to lead the implementation and closeout of the RENEW program, expanding justice-involved services into sustainable care delivery models.

The role involves program operations, care coordination, workflow redesign, implementation planning, and cross-functional collaboration with senior leadership, clinical ops, IT, and community partners to strengthen care beyond the grant period.

Qualifications

  • Bachelor’s degree in public health, Healthcare Administration, Social Work, Nursing, or a related field.
  • Three years of progressively responsible experience in healthcare, public health, or grant-funded programs.
  • Demonstrated experience leading cross-functional projects and implementing operational improvements.
  • Strong project management, organizational, communication, and analytical skills.
  • Experience using electronic health record systems and Microsoft Office applications.
  • Master’s degree preferred.

Responsibilities

  • Lead implementation and successful completion of RENEW program objectives, grant deliverables, and operational milestones.
  • Direct day-to-day program operations while ensuring compliance with grant requirements, organizational policies, and regulatory expectations.
  • Serve as the primary operational liaison with leadership, community partners, and funding stakeholders.

Salary Range: $80,000.00 To $85,000.00 Annually

SUMMARY

The QIF-TJI Program Manager leads implementation and successful closeout of UMMA Health's RENEW Program while advancing the organization's transition of justice-involved services into sustainable care delivery models.

Funded through the Quality Improvement Fund – Transitions in Care for Justice-Involved Populations (QIF-TJI), this position is responsible for program operations, care coordination, workflow redesign, implementation planning, and cross-functional collaboration that support future Enhanced Care Management (ECM), Community Supports, and other CalAIM initiatives.

Working closely with senior leadership, clinical operations, Quality Improvement, Information Technology, consultants, managed care plans, and community partners, the Program Manager develops sustainable operational infrastructure that strengthens care for justice-involved individuals beyond the grant period.

ESSENTIAL DUTIES
  • Lead implementation and successful completion of all RENEW program objectives, grant deliverables, and operational milestones.
  • Direct day-to-day program operations while ensuring compliance with grant requirements, organizational policies, and regulatory expectations.
  • Serve as the primary operational liaison with leadership, community partners, and funding stakeholders.
  • Monitor program performance and proactively resolve operational barriers affecting implementation or participant outcomes.
  • Support strategic planning activities that strengthen long-term sustainability of justice-involved services.
Program Transition & Implementation
  • Lead operational planning activities that transition RENEW into sustainable care delivery models, including Enhanced Care Management (ECM) and future value-based initiatives.
  • Design and implement standardized workflows, documentation standards, referral processes, and operational procedures supporting future contractual requirements.
  • Coordinate implementation activities across Clinical Operations, Population Health, Quality Improvement, Information Technology, consultants, and external partners.
  • Evaluate operational readiness, recommend workflow improvements, and facilitate implementation of approved process changes.
  • Develop implementation resources, workflow documentation, and sustainability plans supporting long-term program success.
Client Services & Care Coordination
  • Provide direct care coordination for justice-involved clients with complex medical, behavioral health, housing, and social needs.
  • Coordinate timely linkage to healthcare, behavioral health, housing, substance use treatment, peer support, and other community resources.
  • Support complex or escalated clients cases while promoting continuity of care and successful community reintegration.
  • Ensure timely, accurate, and complete documentation within the electronic medical record.
  • Identify clients and system barriers affecting access to care and recommend operational improvements.
Team Leadership & Operational Excellence
  • Recruit, supervise, coach, and evaluate Peer Navigators.
  • Establish performance expectations, competency standards, and ongoing professional development activities.
  • Monitor productivity, workflow adherence, documentation quality, and program performance.
  • Develop practical tools, job aids, and training resources supporting consistent service delivery.
  • Foster a collaborative, trauma-informed, strengths-based culture focused on continuous improvement.
Quality Improvement & Performance Management
  • Monitor program performance using operational, quality, and grant metrics.
  • Partner with Quality Improvement staff to support data validation, reporting, evaluation, and performance improvement initiatives.
  • Utilize data to identify trends, recommend improvements, and support rapid-cycle process improvement.
  • Support development of dashboards, implementation tracking tools, and operational reports.
  • Ensure timely completion of grant reporting requirements and participant experience activities.
Community Partnerships & Sustainability
  • Develop and maintain collaborative relationships with Correctional Health Services, manage care plans, community-based organizations, consultants, and other strategic partners.
  • Represent UMMA Health in collaborative meetings, implementation workgroups, and grant learning communities.
  • Strengthen referral partnerships that improve care transitions and participant engagement.
  • Identify opportunities to expand partnerships and support sustainable funding strategies for justice-involved services.
QUALIFICATIONS/POSITION REQUIREMENTS

MINIMUM QUALIFICATIONS
  • Bachelor’s degree in public health, Healthcare Administration, Social Work, Nursing, Human Services, or a related field required; Master’s degree preferred.
  • Three (3) years of progressively responsible experience managing healthcare, public health, behavioral health, care management, quality improvement, or grant-funded programs.
  • Demonstrated experience leading cross-functional projects and implementing operational improvements.
  • Strong project management, organizational, communication, and analytical skills.
  • Experience using electronic health record systems and Microsoft Office applications.

  • Master’s degree in social work (MSW)
  • Bachelor's degree in related field with:
  • Minimum of 2 years of program or project management experience operationalizing grant requirements.
  • Minimum of 2 years of managerial experience, preferably in healthcare or community-based settings.
  • Knowledge of community health services, housing resources, and behavioral health interventions.
  • Compassionate, strengths-based approach to working with vulnerable individuals.
  • Excellent organizational, communication, and problem-solving skills.
  • Report IT issues and coordinate with the IT vendor to resolve them.
Experience and Skills:
  • Strong understanding of reentry services, care transitions, and evidence-based practices for under-resourced populations.
  • Proven ability to develop and sustain community partnerships.
  • Experience in program implementation, data management, and reporting.
  • Knowledge of community health services, housing resources, and behavioral health interventions.
  • Compassionate, strengths-based approach to working with vulnerable individuals.
  • Excellent organizational, communication, and problem-solving skills.
