Job summary

St Johns Community Health is seeking a Re-Entry Program Manager in Los Angeles, CA to lead daily operations of re-entry services and ensure trauma-informed care. The role focuses on staff supervision, grant compliance, and strong partnerships with justice and community organizations.

You will oversee budgets, reporting, and program outcomes while driving continuous quality improvement and collaboration across Medical, Behavioral Health, and Operations teams.

Qualifications

  • Undergraduate degree in social services or related field required; graduate degree preferred.

Responsibilities

  • Oversee daily operations of Re-Entry Services programs and ensure trauma-informed service delivery.
  • Supervise, coach, and evaluate program staff; promote accountability and growth.
  • Monitor performance, deliverables, and outcomes for grant compliance and funding.
  • Perform chart reviews, audits, and quality assurance activities.
  • Manage budgets and expenditures; ensure funds are used per guidelines.
  • Prepare timely reports and data for leadership and funders.
  • Build partnerships with justice agencies, community orgs, and healthcare providers.
  • Coordinate outreach and education to raise awareness of re-entry services.
  • Collaborate with internal departments to provide integrated services.
  • Identify operational challenges and implement process improvements.
  • Support recruitment, onboarding, and retention of staff.
  • Represent St. Johns in leadership meetings and community collaboratives.
  • Develop new grant-funded programs with planning, staffing, and compliance.

Re-Entry Program Manager

Regular Full-Time Los Angeles, CA, US

Salary Range: $74,000.00 To $80,000.00 Annually

Position Summary:

Reporting to the Director of Re-Entry Services, the Program Manager is responsible for the day-to-day management, implementation, and oversight of re-entry programs serving Los Angeles County and surrounding communities. The Program Manager supervises program staff, ensures compliance with grant and contractual requirements, monitors program performance and budgets, and oversees the delivery of high-quality, trauma-informed services for justice-involved individuals. This position works collaboratively with internal departments, community partners, and justice system stakeholders to strengthen referral networks, improve service coordination, and achieve program outcomes. The Program Manager promotes operational excellence, staff accountability, and continuous quality improvement while supporting the organization's mission to improve health, stability, and successful community reintegration for the individuals and communities we serve.

Qualifications/Licensures:

  • Bachelor's degree in Social Work, Public Health, Psychology, Criminal Justice, Human Services, or a related field required; Master's degree preferred.
  • Minimum of three (3) years of experience managing social service, public health, behavioral health, or re-entry programs.
  • Minimum of two (2) years of supervisory or team leadership experience.
  • Experience managing grant-funded programs and ensuring compliance with contractual and regulatory requirements.
  • Knowledge of justice-involved populations, re-entry services, trauma-informed care, housing resources, behavioral health, and community-based services.
  • Experience developing partnerships with community organizations, government agencies, and justice system stakeholders.
  • Strong organizational, project management, and problem-solving skills with the ability to manage multiple priorities.
  • Excellent written, verbal, and interpersonal communication skills.
  • Demonstrated ability to analyze data, prepare reports, and use performance metrics to improve program outcomes.
  • Exceptional attention to detail with the ability to ensure accuracy, maintain compliance, and proactively identify and resolve issues before they impact program operations.
  • Highly proactive and self-motivated with the ability to anticipate needs, take initiative, and drive projects and process improvements with minimal supervision.
  • Proficiency in Microsoft Office Suite and Electronic Health Record (EHR) systems, preferably eClinicalWorks (eCW).

Responsibilities:

  • Oversee the daily operations of assigned Re-Entry Services programs, ensuring high-quality, trauma-informed service delivery.
  • Supervise, coach, and evaluate program staff, promoting accountability, professional development, and a collaborative team environment.
  • Monitor program performance, deliverables, and outcomes to ensure compliance with grant, contract, and organizational requirements.
  • Conduct regular chart reviews, documentation audits, and quality assurance activities to ensure compliance with program standards and funding requirements.
  • Monitor program budgets and expenditures, ensuring responsible use of grant funds and adherence to funding guidelines.
  • Prepare and submit timely program reports, performance data, and other required documentation for leadership and funders.
  • Develop and maintain strong partnerships with justice system agencies, community-based organizations, healthcare providers, and other stakeholders to enhance service coordination and referrals.
  • Coordinate outreach, engagement, and community education activities to increase awareness of re-entry services and improve access to care.
  • Collaborate with internal departments, including Medical, Behavioral Health, Operations, Finance, and Quality Improvement, to ensure integrated, patient-centered services.
  • Identify operational challenges and implement process improvements to enhance program efficiency, service quality, and client outcomes.
  • Support recruitment, onboarding, training, and retention of program staff.
  • Participate in leadership meetings, community collaboratives, trainings, and other activities representing St. John's Community Health.
  • Perform other duties as assigned by the Director of Re-Entry Services.
  • Develop and implement new grant-funded programs and contracts, including operational planning, staffing, budget oversight, policy development, and ongoing monitoring to ensure successful implementation and compliance with funder requirements.
