Qualifications

• Associate’s degree (Bachelor’s degree preferred) in human services or related field.
• Minimum of one–two years providing case management or similar service to vulnerable populations.
• Background check and agency security clearance required where applicable.
• Current tuberculosis documentation and CPR certification required.

Responsibilities

• Assist vulnerable patients in locating and planning access to healthcare services within their community upon release.
• Address questions about navigating the healthcare system post-release.
• Maintain and organize caseload information using Microsoft Office tools.
