Job summary

Port Washington Post Acute – Bremerton, WA is seeking a Registered Dietitian (RD) to assess, plan, implement, and monitor nutrition care for residents in line with federal, state, and facility standards. The RD will collaborate with the interdisciplinary team to optimize nutritional health and outcomes.

The role reports to the Administrator and collaborates with the DNS, Medical Director, and care team. Pay is $40.00–$45.00/hour with flexible scheduling (FT, PT, PRN) and full benefits.

Qualifications

• Assess residents' nutrition status on admission and as needed.
• Develop, implement and monitor individualized nutrition care plans.
• Evaluate nutritional risk, weight changes, wounds, swallowing needs.
• Document progress per facility policies and CMS rules.
• Collaborate with physicians, nursing, therapy, and care teams.

Responsibilities

• Conduct comprehensive nutrition assessments for residents upon admission and as required by regulation or clinical need.
• Develop, implement, and monitor individualized nutrition care plans.
• Evaluate residents for nutritional risk, weight changes, wounds, swallowing concerns, and therapeutic diet needs.
• Monitor and document residents’ nutritional status and progress in accordance with facility policies and regulatory requirements.
• Participate in care plan meetings and collaborate with physicians, nursing staff, therapy, and other interdisciplinary team members.
• Provide nutrition education and counseling to residents, families, and staff as appropriate.
• Review and approve menus to ensure nutritional adequacy and compliance with regulatory standards.
• Monitor food service operations and provide guidance regarding therapeutic diets, food preferences, and nutrition-related concerns.
• Support facility quality improvement initiatives and maintain compliance with state, federal, and CMS regulations.
• Maintain accurate, timely, and complete documentation in the electronic medical record.
