Responsibilities include collaboration with care teams, data entry, and advocating for housing stability while driving up to 50% of the time. Valid driver’s license and insurance required.

Qualifications

• Case management experience.
• Experience with individuals with severe mental illness, substance use disorder, or other disabling conditions.
• Must have a valid driver’s license, vehicle, registration and insurance.
• Desire to be part of a team committed to ending homelessness.

Responsibilities

• Conduct outreach and engage individuals experiencing unsheltered homelessness to determine eligibility and enroll in the program.
• Provide ongoing services through regular in-person appointments, including assessments, care-plan development, service coordination, coaching, education, and referrals.
• Support client engagement by scheduling appointments, providing reminders, coordinating transportation, and addressing barriers to service engagement.
• Collaborate with client care teams to coordinate care, conduct case conferences, and make referrals.
• Maintain accurate documentation for RCC funding compliance and CalAIM/Medi-Cal billing.
• Foster positive relationships with staff, partner agencies, and volunteers to enhance service delivery.
• Assist clients in securing and maintaining housing, providing advocacy and housing-related support.
• Enter data into HMIS and other required databases, ensuring records are up to date.
• Use a team approach to resolve housing barriers and respond to client crises as needed.
• Represent the organization in county-wide meetings and forums as required.
• Provide instruction to improve independent living skills (budgeting, household care, personal care).
• Connect clients to resources for housing stability (employment, income benefits, medical and mental health services, legal aid).
• Participate in team and agency staff meetings.
