Job summary

Chesapeake Regional Healthcare in Chesapeake, VA seeks an experienced Registered Nurse for the Rapid Response/Stroke Team. The role involves assessing patients, coordinating care with ICU/ER teams, and leading discharge planning to improve stroke outcomes.

Minimum 2+ years in ICU/ED/Stroke Unit, with Virginia RN licensure and current BLS/ACLS/PALS certifications. On-site position in a hospital setting with collaborative team dynamics.

Qualifications

• Successful completion of an accredited Registered Nursing program.
• BSN preferred.
• 2+ years’ experience in an Intensive Care setting, Emergency Department, and/or Stroke Unit; recent stroke care experience preferred but not required.

Responsibilities

• Complete a comprehensive nursing assessment based on admission nursing history and appropriate standard of care/protocol.
• Collect data and make interpretations with a nursing diagnosis.
• Initiate and update the standard of care within established time frames; document accordingly.
• Plan and document patient/family teaching and evaluate understanding.
• Initiate discharge planning and continue it throughout hospitalization.
• Maintain data collection, storage, and submission for performance improvement and stroke care standards.
• Serve as liaison between bedside nurse, primary team, and ICU/ER team to improve outcomes.
• Assist in care of all emergency Stroke/Rapid Response Team alerts per policy.
