Qualifications

Responsibilities

• Leads design and development of interprofessional processes for plan of care.
• Provides Evidence-Based Practice (EBP) care to patients and caregivers.
• Defines outcomes aligned with interprofessional team goals and cost considerations.
• Identifies communication strategies among individuals, family, community and social systems.
• Provides leadership for establishing, improving, and sustaining collaborative relationships to ensure safe quality care.
