Catholic Charities, Diocese of Trenton is hiring a Case Manager (Rapid Rehousing) in Trenton, NJ. You will deliver case management/counseling services to clients and families, design individualized plans, assess needs, and coordinate access to housing and related supports.

The role requires a relevant degree or comparable experience, plus state- required credentials where applicable. The position is full-time, non-exempt, 35 hours per week, with standard hours and a Monday/Wednesday/Friday

Qualifications

Minimum qualifications include a Bachelor’s degree in a related field or equivalent mix of coursework and experience.
Experience in human services and a valid driver’s license are required.
Credentialed professionals (CPRP, CRC, CADC) with related experience are preferred.

Responsibilities

Design, implement, and monitor individual program plans for clients.
Assess client needs and eligibility for services.

Maintain client records and required documentation per standards.

Job Summary

Responsible for delivering case management/counseling services for clients and families.

Essential Functions

Designs, implements and monitors individual program plans for clients.
Assessments client needs; investigate their eligibility for services.
Counsels and aids individuals and families requiring agency assistance.
Maintains client records and completes required documentation according to agency and accreditation standards. This may include, but is not limited to data entry, progress notes, activity reports, time sheets and logs.
Coordinates client activities.
Assists clients with accessing other services (job training, counseling, housing, medical services, etc.)
Networks with other providers to generate community resources for participants.
May provide supportive counseling in individual and group settings.

Other Duties

Other related program tasks as assigned by the Program Director. Willingness and ability to work flexible hours if needed.

Position Requirements

Must also have basic data entry knowledge, including the ability to accurately enter, update, and maintain client, program, and service‑related information in electronic databases or tracking systems.
