Job summary

Ethica Health is seeking a Licensed Practical Nurse (LPN) in Georgia to support patient assessments and care planning in line with federal/state requirements. You will coordinate with families and the interdisciplinary team to ensure care plans meet clinical needs and patient wishes.

You will perform periodic assessments and oversee the MDS process, ensuring accuracy and compliance while maintaining strong communication with patients and colleagues.

Qualifications

• Demonstrated ability to communicate effectively orally and in writing.
• Ability to maintain effective working relationships with patients, families, department heads and colleagues.
• Knowledge of data entry, MDS coding and the RAI process.

Responsibilities

• Assist with development and completion of patient assessments following federal/state requirements and organizational goals.
• Coordinate patient/family assessment interviews and develop care plans aligned with clinical needs and patient wishes.
• Schedule and coordinate meetings with family and interdisciplinary team members to discuss care plans and interventions.
• Perform periodic patient assessments and monitor the MDS process for accuracy.

Job description

• Assists with the development and completion of patient assessments in accordance with federal and state requirements and organizational goals and guidelines.
• Conducts and/or coordinates patient and family assessment interviews and develops care plans consistent with patient clinical needs, risk areas, and wishes.
• Schedules and coordinates meetings with family members and interdisciplinary team members to discuss patient care plans, special needs, and/or appropriate interventions.
• Performs periodic patient physical assessments and monitors the Minimum Data Set (MDS) process for accuracy.

Requirements:

To be considered for this opportunity applicants must:

• Have a current license as a Licensed Practical Nurse from the Georgia State Nursing Board and demonstrated knowledge of data entry, MDS coding, and entire RAI process.
