Job summary

Valley-Wide Health Systems, Inc. seeks a compassionate RAC Aide to support individuals facing mental, emotional, behavioral, or substance use challenges within the Regional Assessment Center.

You will provide hands-on care, crisis intervention, and collaborate with clinical staff to ensure a safe, therapeutic environment. Responsibilities include monitoring in the Transitional Residential Treatment Unit, coordinating meals, assisting with daily living activities, and documenting care per state

Qualifications

  • High School Diploma-equivalent required.
  • Minimum of 2 years’ experience in healthcare, human services, or customer service.
  • BLS certification provided upon employment.
  • Colorado driver’s license and willingness to travel as needed.
  • Must pass Behavioral Health background investigation.

Responsibilities

  • Provide care, support, and monitoring to individuals in the TRT.
  • Collaborate with RAC team members and Valley-Wide staff for coordinated care.
  • Complete daily RAC tasks: meals, cleaning, daily living assistance, calls, unit duties.
  • Document accurately per agency and state standards.
  • Assist develop and implement individual care plans with clinical staff.

Join Our Team at Valley-Wide Health Systems, Inc.Valley-Wide Health Systems is seeking a compassionate and committed RAC Aide to support individuals experiencing mental, emotional, behavioral, or substance use challenges. The ideal candidate will provide hands-on care, crisis intervention, and support within the Regional Assessment Center (RAC), working collaboratively with clinical staff to ensure a safe and therapeutic environmentKey Responsibilities:Responsibilities & Essential Functions:Provide care, support, and monitoring to individuals in the Transitional Residential Treatment Unit (TRT).Collaborate with fellow RAC team members and other Valley-Wide staff to ensure coordinated carePerform daily RAC tasks including meal coordination, facility cleaning, assistance with daily living needs, phone communication, and other unit dutiesComplete accurate and timely documentation consistent with agency and state standardsAssist in the development and implementation of individual care plans in collaboration with clinical staffDeliver individual and group behavioral health interventions for individuals in the ATUParticipate in team meetings, individual supervision, and clinical education opportunitiesMaintain compliance with HIPAA, 42-CFR, and agency confidentiality policiesAssist with clinical assessments, crisis interventions, therapy sessions, case management, and outreach under the direction of a clinical supervisorDevelop and monitor treatment plans, ensure daily progress notes are completed before shift endComplete Treatment Plans within 24 hours of admissionObtain and maintain QMAP certificationSuccessfully complete all required training within designated timeframesSupport culturally competent care and documentationProvide crisis services and client transport when needed, including outside of normal hoursQualifications & Skills:High School Diploma or Equivalent requiredMinimum of 2 years’ experience in healthcare, human services, or customer serviceStrong communication, organizational, and problem-solving skillsBLS certification (provided upon employment)Colorado driver’s license and willingness to travel as neededMust pass Behavioral Health background investigationHigh level of accuracy, attention to detail, and ability to multi-taskFamiliarity with office equipment and strong computer skills, especially Excel and Microsoft programsExperience with peer support, recovery services, or behavioral health preferredWhy Join Valley-Wide?Valley-Wide Health Systems, Inc. is dedicated to providing high-quality, patient-centered healthcare to all individuals across our multi-county service area in southern Colorado.
