Qualifications

• Experience with vulnerable populations or behavioral health is preferred.

Responsibilities

• Identify members at risk for medication non-adherence using pharmacy claims and internal reports.
• Conduct telephonic outreach to assess medication adherence and identify barriers.
• Educate members on importance of medication adherence and disease self-management.
• Assist with closing member barriers.
• Develop and present a report to the interdisciplinary team on barriers and solutions.
• Communicate with individuals across the care continuum and document in EMR.
• Engage with member/family to discuss care needs and information.
