The role focuses on turning data into action to improve patient outcomes and practice performance, supporting PCMH initiatives and quality improvement activities. You will collect, analyze, and interpret data, coordinate outreach, and work closely with providers and staff to enhance patient experience and care continuity in a collaborative team

Qualifications

• Minimum requirements include a high school diploma or equivalent.
• Experience in hospital, healthcare, or medical office setting preferred (2+ years).
• Proficiency with computer systems and electronic health records; medical terminology knowledge.

Responsibilities

• Utilize EMR data, patient registries, and reporting tools to identify trends and support outreach and quality improvement.
• Analyze workflows and coordinate data-driven improvement projects with providers and staff.
• Track PCMH activities, prepare reports, and assist with data submissions and patient engagement.
