Northeastern Vermont Regional Hospital in St. Johnsbury, VT is seeking a Patient Outreach & Quality Coordinator to transform data into actions that improve community health and patient care outcomes.

You will analyze trends, support quality initiatives, and assist with PCMH activities while coordinating patient transitions and ensuring accurate record handling and provider panel management. This role blends healthcare operations with data analytics in a collaborative team.

Formación

• High school diploma or equivalent required.
• Minimum of two years of experience in a hospital, healthcare, or medical office setting preferred.
• Experience using computer systems and electronic health records.
• Knowledge of medical terminology.
• Ability to manage multiple priorities and remain calm and professional in a fast-paced environment.
• Strong written and verbal communication skills.

Responsabilidades

• Identify trends using EMR data, patient registries, and reporting tools to monitor quality measures and support outreach.
• Lead data-driven improvement projects, coordinate quality meetings, and collaborate with providers and staff to optimize patient care and efficiency.
• Support PCMH activities by tracking quality measures, preparing reports, coordinating data submissions, and assisting with patient engagement.
• Coordinate patient transfers into and out of the practice, ensure timely release of medical records, manage provider panels, and communicate with patients and external organizations.
