Via Care LA is seeking a Licensed Vocational Nurse (LVN) to join our Quality Department in Los Angeles, CA. The role centers on connecting providers, clinic teams, and quality initiatives to ensure patients receive right care at the right time, with a focus on HEDIS and star ratings.

You'll monitor preventive screenings, chronic disease metrics, and care gaps in the EHR, collaborating with managers, providers, and outreach teams to drive improvements and support 5-star IPA ratings across our

Formación

• Graduate of an LVN or LPN program and licensed in California (BVNPT).
• Current BLS certification for healthcare providers.
• Experience in outpatient or primary care quality settings is preferred.

Responsabilidades

• Assist data collection for quality programs and audits.
• Identify and pull care gaps from the EHR to track preventive and chronic metrics.
• Prepare bulleted reports for leadership and committees.
• Collaborate with clinic teams to close gaps and improve care quality.
