Job summary

Qualifications

• Graduate of an accredited LVN or LPN program.
• Active LVN license in California.
• Basic Life Support (BLS) certification.

Responsibilities

• Monitor HEDIS and star ratings metrics to drive performance.
• Identify and address care gaps in the EHR to improve preventive care.
• Collaborate with clinic teams to implement quality initiatives.
• Assist with data collection and reporting for quality programs.
• Participate in audit readiness activities and regulatory compliance.
• Translate audit data into clear leadership reports.
