Qualificações

• Experience as a Medical Assistant.
• Certification as a Medical Assistant is required.
• Comfort with clinical data, charts, and registries.
• Interest in population health and quality improvement.

Responsabilidades

• Audit patient charts to identify care gaps and outreach opportunities.
• Maintain and review patient registries for preventive care and chronic disease management.
• Support population health initiatives and value-based care programs.
• Help track and improve clinical quality measures and outcomes.
• Review payer information and use payer portals to identify and close quality gaps.
• Participate in payer meetings to maximize quality improvement opportunities.
• Assist with audits and follow-up related to quality improvement activities.
• Educate staff, providers, and residents on quality measures and improvement efforts.
• Participate in Continuous Quality Improvement and Risk Management activities.
• Provide MA coverage in Primary Care when needed.

Greater Seacoast Health is looking for a Quality Improvement Medical Assistant to join our Quality and Population Health team in Somersworth.

This isn't your typical MA position.

Instead of spending every day in an exam room, you'll use your clinical knowledge to help identify and close care gaps, review patient charts and registries, support preventive and chronic disease management, and help our teams improve quality outcomes across the organization.

You'll still be a Medical Assistant - but you'll have an opportunity to use that experience in a different way.

What You'll Do

You'll work closely with our Quality and Population Health team to help make sure patients are receiving the preventive and ongoing care they need.

Your work will include:

• Reviewing and auditing patient charts to identify care gaps and opportunities for patient outreach
• Maintaining and reviewing patient registries for preventive care and chronic disease management
• Supporting population health and value-based care initiatives
• Helping track and improve clinical quality measures and outcomes
• Reviewing payer information and working within payer portals to identify and close quality gaps
• Participating in payer meetings and helping maximize quality improvement opportunities
• Supporting audits and follow-up related to quality improvement activities
• Helping educate staff, providers, and residents on quality measures and improvement efforts
• Participating in Continuous Quality Improvement, Chronic Disease, and Risk Management activities
• Occasionally providing Medical Assistant coverage in Primary Care when needed
