Job summary

Quality Care TCM Coordinator

The Quality Care TCM Coordinator (QCC) is responsible for coordinating aspects of patient care followingahospital,short-termnursingfacilityorrehabdischarge,includingworkingdirectlywithourpatient population and their families, physicians, clinicians, and other medical personnel.

Key Responsibilities:

Essential Functions

  • Demonstrates, through behavior, Center for Primary Care’s Mission Statement.
  • Supports quality improvement activities including data management and reporting, health insurance incentive programs, and other value-based reimbursement programs.
  • Monitors key internal and external clinical quality metrics reports, and other performance reports.
  • Collaborates with practices to identify opportunities for improvement and develops supportive plans to target opportunities using quality improvement methods and tools.
  • Assists in providing education for staff and providers on quality and population health initiatives to maintain or improve performance.
  • Supports clinical data collection, completes clinical chart reviews and uploads chart records for: Medicare, Medicare Advantage, and other insurance programs, as indicated.
  • Provides support for the practice to meet organizational quality and efficiency goals through optimizing provider and patient engagement.
  • Communicates with local specialist offices or other entities to coordinate care and ensure patient follow up and record collection.
  • Completes Patient Assessment Forms for all eligible insurance plans and tracks progress for completion and payment.
  • Attends planned meetings scheduled by the Manager of Quality Care Coordination.
  • Communicates with DCM regarding physician inquiries, requests, and/or concerns.
  • Communicates with the physicians regarding patient concerns and physician preferences for scheduling.
  • S upportsqualityimprovementbyguidingpatientsastheytransitionfromaninpatientfacility,suchasa hospital, rehab, or short-term nursing home to their home, domiciliary or assisted living facility.
  • Preppingvisitnotesforupcomingtransitionalcaremanagement visits.
  • Ensuringthatalldocumentsrelatedtopatient’sdischargehavebeenobtainedandscannedintochartpriorto visit.
  • Fieldingincomingcallsfromareahospitalsandroutingmessagestotheappropriatelocationsforschedulingof needed tests.
  • Monitoringandreviewingpost-visitreportsforpatientswhohavebeenseenfortransitionofcare visits.
  • Attendingplannedmeetingsasdeemednecessary by supervisor.
  • Communicatesmanagerregardingphysicianinquiries,requests, and/or concerns.
  • Communicateswiththephysicianswhenneeded, regardingpatientconcernsorwithothermattersasthey arise.
  • Communicateswithlocalspecialistofficesorotherentitiestocoordinatecareandensurepatientfollowupand record collection.
  • Assistsinproviding educationforstaff andprovidersonTCMinitiativestomaintain orimprove performance.

Qualificationsfor Success:
Education, License/Certification, and Experience Requirements

Education: High School Diploma or GED required.

License/Certification: LicensedPracticalNurse (LPN)orCertifiedMedicalAssistant required.

Experience: At least two (2) years of recent clinical experience. Experience with AthenaOne strongly preferred.

  • Strongworkingmedicalknowledgeofthe clinical needsanddiseaseprocessesforthechronicallyill population.
  • Excellenttelephone,oral andwrittencommunication skills.
  • Abilitytoworkautonomously andbedirectly accountablefor assignments.
  • Strongcomputerskills,includingMicrosoftofficeandEMR experience.
