Are you a skilled RN with a sharp eye for detail and a passion for quality care? Join our team as a Quality Care Manager and help ensure that every patient receives exceptional, compliant, and effective care.

Compensation starting at $80,000/year. Open for discussion!

What You’ll Do

  • Review patient documentation and care plans to ensure accuracy and compliance with regulations.
  • Confirm medical necessity, skilled need, and alignment with physician orders and homebound status.
  • Approve and finalize Plans of Care and OASIS assessments on time.
  • Communicate with clinicians to correct documentation issues and support best practices.
  • Collaborate with the Quality Director on quality improvement initiatives.
  • Use SHP data and reports to improve outcomes and performance.
  • Participate in case conferences and help cover weekend/holiday tasks as needed.
  • Stay current with home health regulations, CHAP standards, and OASIS guidelines.

What You Bring

  • Active, unencumbered RN license.
  • Graduate of an accredited School of Nursing.
  • At least 1 year of home health experience.
  • Strong working knowledge of OASIS, Medicare Conditions of Participation, and Title XXII.
  • Detail-oriented with great communication and problem-solving skills.
  • Proficient in EMR systems; Home Care Home Base experience is a plus.
  • Ability to work independently and manage time effectively.
