We are hiring for a Patient Care Manager RN in Hospice.

This is a full-time, salaried position that includes both managerial duties and home visits for hospice care.

Home-based hospice experience and at least 4 years of RN experience are required.

Responsibilities

The Patient Care Manager - Hospice supports the Executive Administrator/Director of Clinical Operations by overseeing patient care activities to ensure safe, efficient, and compliant care that meets nursing, regulatory, medical, and payer standards.

• Ensure compliance with hospice policies, procedures, and all local, state, and federal regulations.
• Promote and oversee quality of care and safety for patients and staff.
• Investigate complaints and resolve problems thoroughly.
• Provide staff in-services to promote health, safety, regulatory compliance, and address educational needs.
• Requires at least 4+ years of full-time RN experience or equivalent.

Education and Experience

License Requirements

• Current RN licensure in the state of practice and at least one year of clinical experience.
• Current CPR certification.
• Valid driver's license, vehicle insurance, and access to reliable transportation or public transit.

Additional State-Specific Requirements

• VA: Experience with terminally ill patients is required.
