Job summary

The W Group, New York State's largest provider of senior living and adult care, seeks a Quality Auditor to join our compliance team. This field-based role conducts audits across multiple facilities, gathers evidence, and prepares reports to support corrective actions and survey readiness.

You will work under the Compliance Officer, travel to ACF, ALP, LHCSA sites, build professional relationships with facility staff, and help maintain survey readiness with thorough documentation and trend

Qualifications

• Bachelor's degree required or equivalent in healthcare field.
• 1–3 years experience in assisted living, ACF, LHCSA, or healthcare compliance operations.
• Familiarity with NYS DOH survey processes and regulatory requirements.
• Prior experience conducting audits, inspections, or structured compliance reviews preferred.
• Proficiency in documenting and reporting audit findings.

Responsibilities

• Conduct scheduled internal compliance audits across multiple facilities using standardized tools.
• Gather objective evidence through documentation review, observation, and interviews.
• Complete audit reports and submit findings to the Compliance Officer before leadership review.
• Perform re-audits to verify corrective actions and monitor Plans of Correction.
• Maintain organized audit records in ALIS and assist in trend reporting.

Quality Auditor

Type: Full-Time | Field-Based

Location: Brooklyn, NY -- travel required across all W Group facilities

About This Role

The W Group -- New York State's largest provider of senior living and adult care -- is looking for a Quality Auditor to join our growing compliance team. This is a field-based, multi-facility role that sits at the heart of our internal compliance and quality improvement program. You'll work directly under our Compliance Officer, conducting structured audits across our ACF, ALP, and LHCSA facilities to identify gaps, support corrective action, and help keep our facilities survey-ready.

This role is for someone who takes compliance seriously but approaches it as a partner -- not an inspector.

What You'll Do

Audit Execution

• Conduct scheduled internal compliance audits across all W Group facilities using standardized tools and checklists developed by the Compliance Officer

• Gather objective evidence through documentation review, direct observation, and structured staff interviews

• Complete thorough audit reports documenting findings and observations; submit to the Compliance Officer prior to any communication to facility leadership

Follow-Up & Validation

• Conduct re-audits to assess the effectiveness of corrective actions

• Verify that Plans of Correction are being carried out as written

• Flag corrective actions that are incomplete, ineffective, or not yet implemented

Documentation & Trending

• Maintain organized audit records in our centralized tracking system (ALIS)

• Identify recurring findings and cross-facility patterns for the Compliance Officer

• Assist in compiling audit data for compliance trend reports

Survey Readiness

• Support DOH survey readiness activities at facilities as directed

• Identify documentation gaps and operational vulnerabilities ahead of surveys

• Provide on-site support during active DOH surveys as needed

Facility Engagement

• Build respectful, professional working relationships with facility administrators and staff

• Communicate audit scope and purpose clearly at each site visit

• Refer all regulatory interpretation questions to the Compliance Officer

What We're Looking For

• Bachelor's degree required; Healthcare Administration, Nursing, Social Work, or related field preferred

• 1-3 years of experience in assisted living, ACF, LHCSA, or healthcare compliance operations

• Familiarity with NYS DOH survey processes and ACF/ALP/LHCSA regulatory requirements

• Prior experience conducting audits, inspections, or structured compliance reviews preferred

• Strong written communication and documentation skills

• Highly organized and self-directed -- comfortable managing an independent field schedule

• Proficiency with electronic records systems; ALIS experience a plus

• Valid driver's license and reliable transportation required
