Job summary

Harbor Healthcare System in the Portland area is seeking a part-time Quality Assurance LVN/LPN to ensure quality health services and review documentation for compliance. The candidate must hold an active LPN/LVN license, have hospice or home health experience, and be able to travel as needed.

The role focuses on reviewing nursing notes, POC and re-certifications, and assisting with pre/post audit forms while collaborating with multidisciplinary teams. Strong communication skills are required.

Qualifications

• Must have an associate degree (A.A.), or equivalent from a two-year college.
• Must have at least one year’s experience in hospice, home health or a related field.
• Must have a current State LPN or LVN license.
• Must be able to speak and write effectively.
• Must work well with other disciplines to coordinate patient care.
• Must be able to prepare forms for pre and post payment audits.
• Must have a current State driver's license
• Experience in reviewing nurses’ notes for completeness, compliance with physicians’ orders, authorized services, regulations required
• Experience in reviewing admission packets required
• Experience required in reviewing all POC and re-certification for: accuracy, order updates, frequency updates, goal updates, medication profiles review, format and wording, diagnoses update, C.N.A. care plan
• Conducts one-on-one in-services for documentation problems as needed
• Effective written and verbal communication
• Ability to travel - must havereliable transportation
• Clean background and drug screen
